Tower Hamlets PCT and the London Borough of Tower
Hamlets have developed a joint 10 year strategy for
improving health and well being in Tower Hamlets. This
strategy envisages a new model of integrated and
modernised services to be available near peoples' homes.
The first of these developments consists of a primary
healthcare centre plus housing.
Description
The developments offers an excellent opportunity to provide an
innovative approach to primary care by using teams of healthcare
professionals that focus on planned care and urgent / unplanned care, in
separate ways.
The aim is to provide a health as well as an illness service, where every
interaction with a patient becomes an opportunity to improve their long
term health. We also wanted to make the illness service as high-quality as
possible. To do this, we wanted to focus on consultations, on getting
clinician-patient contacts right. The building will help do this by facilitating
the sorts of contacts and consultations we are striving for.
The building design and model of care works against the inverse-care law
usually in operation in health services: reversing the usual system of the
least needy using the most services.
The building will be welcoming, especially to those with the highest health
needs, including the elderly and parents of young children, and fit in well
with the local community. The Centre will have active links with, for
example, the school and children’s centres and other community and
social services and other outside agencies.
